Back Pain: When Should You See a Physiotherapist?
Back pain is one of the most common reasons people see a healthcare professional in the UK. Most of us will experience it at some point. And most of the time, the advice is the same: keep moving, take pain relief, and wait for it to settle.
For many people, that works. But for a significant number, the pain does not settle. It becomes persistent, starts affecting sleep, limits what they can do at work, or begins to radiate into the leg. That is when people start to wonder whether they should see someone, and who that someone should be.

When back pain is likely to settle on its own
Most acute back pain, the kind that comes on after lifting something awkward or sitting in the wrong position for too long, will improve within a few days to a few weeks. During that time, the most helpful things you can do are:
Keep moving as normally as you can, even if it is uncomfortable
Avoid prolonged bed rest
Use over-the-counter pain relief if needed (always follow the guidance on the packaging)
Apply heat or cold to the affected area
This is not the moment when physiotherapy is urgent. If your pain is manageable and improving steadily, watchful waiting is reasonable.
When you should see a physiotherapist
There are specific situations where a physiotherapy assessment is the right step, and the sooner the better.
Your pain is not improving after 2–3 weeks
Back pain that lingers beyond three weeks without clear signs of improvement is unlikely to resolve fully on its own. A physiotherapy assessment can identify what is maintaining the problem and start you on a structured recovery plan.
Your pain is spreading into your leg
Pain, tingling, or numbness that travels from the back into the buttock, leg, or foot is often a sign of nerve involvement, most commonly sciatica. This needs a clinical assessment. A physiotherapist can identify which nerve is affected, what is causing the irritation, and which exercises and treatments are most likely to help.
You have had the same problem before
Recurrent back pain often has an identifiable pattern, a specific movement or posture that loads the spine the wrong way. Physiotherapy can identify that pattern and build the strength and control to break it.
Back pain is affecting your sleep or daily function
If you cannot find a comfortable position in bed, are struggling at work, or have reduced what you are doing because of pain, that is a signal that the problem is not resolving naturally and needs assessment.
You want to understand what is actually wrong
A physiotherapy assessment gives you a clinical diagnosis, an explanation in plain terms, and a clear picture of what will help. Many patients find that understanding the problem significantly reduces the anxiety that often comes with back pain.
What a physiotherapist can do for back pain
Physiotherapy for back pain is not one thing. It depends entirely on what the assessment finds.
Treatment may include manual therapy to restore movement in stiff joints, soft-tissue work to reduce muscle tension, exercise programmes to build the spinal stability and strength that protect against recurrence, education about posture and movement, acupuncture for pain management, and advice on activity modification during recovery.
The goal is not just to reduce your pain in the short term. It is to understand why it happened, address the underlying cause, and reduce the likelihood of it happening again.
